This exciting development is available in one of our newest regions - Bundaberg. We are looking for a Team Leader Care Coordination with as much passion as we have in our mission to deliver high quality stepped care solutions to the community, particularly in suicide prevention and more severe presentations of mental health. 

Reporting directly to the Mental Health Services Manager, your primary responsibility will be to provide Clinical care coordination/liaison and Clinical Nursing Services. You will also be required to liaise closely with family and carers as appropriate, regularly review the persons mental state and administer prescribed medication, monitoring any potential side effects. 

 About YOU:

  • Current Nursing and Midwifery Board of Australia Certificate of Registration
  • Credentialed with the Australian College of Mental Health Nurses (ACMHN)
  • You will need to have had practical experience and knowledge in client and risk management, working with vulnerable clientele from diverse backgrounds.
  • Demonstrated ability to practice within a recovery framework and early intervention framework (when working with young people) in accordance with the Standards of Practice For Mental Health Nursing
